CONSENT FORM

PURPOSE: You are being asked to participate in a research study. The purpose of the study is to better understand how teams make decisions. 

PROCEDURES: If you decide to participate, we will ask you to complete a series of business decision-making tasks with a team and respond to a few survey questions in a questionnaire. The total time to participate will be approximately 40 minutes. When responding to the questionnaire, we ask that you keep in mind that there are no right or wrong answers. We are simply interested in understanding your experiences. Your activities will be recorded. If you decide to participate, you are agreeing to be recorded.

 
RISKS: There are no anticipated risks involved in participating in this study. If you are uncomfortable with the questions you may choose to skip questions that cause you discomfort or you may discontinue participation at any time. 

BENEFITS: There are no direct benefits to your participation in this survey other than gaining the experience of research participation. 

CONFIDENTIALITY: You may participate in some activities with other people. All group activities during this session will be recorded on digital video. All records and videotapes from this study will be stored securely and kept private. The video recordings will be coded so that no personally identifying information is visible on them. The recordings will be erased after they are transcribed and coded. All responses that you provide in this study will be kept confidential. No identifying information will be associated with your responses; your responses will be linked to a random unique identification number and will not be stored with identifying information. All study data will be kept on password-protected computers. The data and video/audio recordings will be viewed only for research purposes by the primary investigator and co-investigators. 

 
RIGHT TO REFUSE OR WITHDRAW: You may refuse to participate or change your mind about being in the study and quit after the study has started. Besides, the investigator may withdraw you from the study at any time.
